i need to fax my designation documentation to my credit card company, like my social indemnity number, my name contained by birthday, and utility bill, can you guys tell me how to start, where on earth should i put my name contained by address and theire fax number.

Never fax that nature of information. Get a physical address and mail it overnight express near a signed receipt.

On a blank piece of paper, include your describe, address, account number, fax number, etc (basically adjectives the info needed in lay down to identify you)

Then put ATT (attention) to whomever your faxing it to.

Then put a short explaination of the fax.

Example: I'm faxing this to Visa to prove that I have the current resident at this address.
